Go to app.helpyousponsor.com
1. Click "Program Options"
Click on the "Program Options" button.
2. Click "Program Forms"
Navigate to the "Program Forms" section.
3. Click "Manage Fields"
Access the "Manage Fields" menu.
4. Click "Edit"
