1. Click Program Options

This takes you to the program options page

2. Click "Program Forms"

Click on "Program Forms"

3. Click Create A Form

Click this button to create a form

4. Click here

Click the "Enter Form Name" form field

5. Fill "report name"

Fill in "report name"

6. Click the select or drop down

Use the drop down to select the options

7. Click "Progress Report"

Select the "Progress Report" option

8. Click "Save"

Choose the "Save" option
