To create a progress report, first you have to create a program form and add fields to it. To know more about program forms click here.
When creating a progress report form, choose type of "Progress Report".
Then after that you'll have to add fields onto the form, these fields will have to be in accordance to what you want to keep track of. To see how to add fields to a form click here.
